Welcome to Rocky Mountain Spine and Sport.
Here are some things you need to know before you start.

What to expect: Expect friendly, courteous and professional care from our entire staff. Your Physical Therapist will spend the first 10 to 15 minutes taking a detailed medical history and conducting a thorough physical therapist examination. Some conditions do require more time than others to fully evaluate. Following the examination your Physical Therapist will review the findings with you, answer any questions or concerns you may have and discuss the plan of care. A frequency and duration will be recommended based on the findings in the examination to effectively manage your condition with goals in mind. Your Physical Therapist may, on your first visit, instruct you in a home exercise program. These home exercises are crucial to your progression and are part of your plan of care. Performing these exercises as instructed will speed the recovery process. Once again, there is always time for questions if you are having trouble with your plan of care. You will have the same Physical Therapist for the duration of your treatment in order to ensure continuity and quality of care.

Appointments: We usually offer appointments within 24 hours and in some cases, same day appointments are available.

Your first visit: Please show up 15 minutes before your scheduled appointment time. We have some paperwork that needs to be filled out prior to your evaluation and treatment. Click below on the New Patient Forms to download your Patient History Form and Consent of Care & Treatment Form to fill out and bring to your appointment. If you don't have Acrobat Reader you can download it here free of charge.

What to bring: If you have been referred by a Physician please bring your insurance card, driver's license and the prescription, if one was given to you. Some Physicians that refer to us regularly fax prescriptions to us.

What to wear: Comfortable clothing and shoes are recommended. If we are treating an extremity we will need to expose the area. For neck and back injuries we will also need to expose the area.

Payment Options: In most cases, we will bill your insurance company for your treatments. We also have very affordable cash pay programs - Please call our office for details. If you desire a payment plan for in or out-of-network deductibles, we will kindly set this up at your request. Visa and MasterCard accepted.
